3-phase roadmap (3 to 6 months)
Phase 1: Foundation (Weeks 1 to 4)
- Baseline routines: food pattern, movement, sleep, stress, alcohol (if relevant)
- Pick 1 to 2 priorities and create a simple weekly rhythm
- Simple tracking for awareness and follow-through
Phase 2: Build (Weeks 5 to 12)
- Strengthen meal planning and movement consistency
- Strategies for travel, social events, stress spikes, and busy weeks
- Support follow-through on clinician plan and timing for check-ins
Phase 3: Maintain (Weeks 13 plus)
- Relapse prevention and habit resilience
- Maintenance plan that works across seasons
- Prep for repeat labs or follow-up visits ordered by your clinician
Optional HSA/FSA reimbursement (Letter of Medical Necessity)
Some plans may reimburse coaching when it is part of a medical care plan and supported by a clinician Letter of Medical Necessity. Coverage is plan-dependent and not guaranteed.
- Ask your plan administrator if coaching is reimbursable with an LMN for your diagnosis.
- Send the LMN template to your clinician’s office.
- Keep itemized receipts and submit per your plan’s process.

- Type of service: Health and wellness coaching (behavior change and adherence support)
- Scope: education, habit design, accountability, follow-through support
- Not included: diagnosis, prescribing, medical nutrition therapy, lab interpretation
- Typical duration: 3 to 6 months (weekly or biweekly sessions)
- Client consent: progress summaries can be shared with the care team when requested
If you choose to provide an LMN, plans commonly expect diagnosis, date, duration, frequency, and a brief statement of medical necessity.

Dear Swati,
Based on our long drawn friendship which began in Stuttgart, Germany , I approached you as a friend seeking advise for my menopausal health issues and you not only gave me plenty of it but also backed it with nutritional facts and figures . Our sessions were extremely rewarding as you listened so very patiently, noted every detail and explained things to me which made me see things from a totally different perspective.
