Phase 1: Foundation (Weeks 1 to 4)

  • Pick 1 to 2 priorities and a weekly rhythm
  • Simple tracking for awareness and follow-through

Phase 2: Build (Weeks 5 to 12)

  • Strengthen meal planning and movement consistency
  • Stress, travel, and restaurant strategies
  • Support follow-through between lab check-ins

Phase 3: Maintain (Weeks 13 plus)

  • Relapse prevention and long-term routine design
  • Maintenance plan that fits real life
  • Prep for follow-ups ordered by your clinician
Optional HSA/FSA reimbursement (Letter of Medical Necessity)

Some plans may reimburse coaching when it is part of a medical care plan and supported by a clinician Letter of Medical Necessity. Coverage is plan-dependent and not guaranteed.

  1. Ask your plan administrator if coaching is reimbursable with an LMN for dyslipidemia or hypertriglyceridemia.
  2. Send the LMN template to your clinician’s office.
  3. Keep itemized receipts and submit per your plan’s process.
